Going on Holiday?

For patients travelling overseas the practice runs a Travel Clinic offering travel advice, medication to prevent Malaria and relevant vaccinations.

If you are planning to travel abroad, especially outside Europe, please book an appointment at the travel clinic for advice. A charge will be made for services outside the NHS. The practice is a registered Yellow Fever Vaccine centre. Please aim to contact the surgery at least eight weeks before travel to allow the vaccinations to be organised and to allow the immunity to take effect.

Prior to attending the Travel Clinic please collect and complete a Travel Clinic questionnaire detailing the areas of intended travel. Alternatively you can download a questionnaire HERE

This information is used to provide geographically specific travel advice. Once this form is returned to reception a Travel Clinic appointment can be made. Please allow plenty of time before your planned departure date.
